Application for IV-D Services

The FOC program operates under the Social Secuirty Act Title IV-D Child Support and Establishment of Paternity.  Part D, Section 4.5

The following are the options available to file for IV-D Services in Michigan:

1) You may pickup the DHS-1201 form from the Friend of the Court Office or print it here and mail it to the address listed on the form.

2) You may complete an electronic “e1201” form by accessing the MiChildSupport portal at www.micase.state.mi.us or go to any MDHHS Office and accessing the kiosk.

3) You may go to the MDHHS website at www.michigan.gov/mdhhs to complete the paper DHS-1201.
