The following are the options available to file for IV-D Services in Michigan:
1) You may pickup the DHS-1201 form from the Friend of the Court Office or print it from the website listed below and mail it to the address listed on the form.
2) You may complete an electronic “e1201” form for IV-D Services online by accessing the Mi-Case portal at www.michigan.gov/micase or go to any DHS Office (Department of Human Services) and access the kiosk at the office to file the “e1201 form”.
3) You may go to the DHS child support website at http://www.michigan.gov/dhs to complete the paper DHS-1201.
